An employee and a contracted forklift operator were moving matting boards that had been stacked in two piles. The top matting board in the shorter pile was forward and off center, leaving a gap between that mat and the taller pile of mats behind it. The employee was standing in the gap. When the forklift extended forward the lifting apparatus became caught on the off-center matting board and pushed it forward. The employee's right ankle was pinched between this mat and the pile of mats behind it
